Better understand this increasingly prevalent approach to improve the delivery of quality health care

To All Federation Members. Recently the Ministry of Health has released a survey to get a better understanding of the breadth of case management, and care co-ordination.  The Ministry is presently surveying the addiction, mental health, community support sector, CCACs, and other health systems to better understand this increasingly prevalent approach to improve the delivery of quality health care.  It is crucial that the work of addiction and mental health providers is fully understood by the Ministry as it looks at these issues.   Originally this survey had a deadline of May 8th.  The Ministry has extended this deadline until May 15th, 2009.  The Federation would encourage organizations to complete this survey and ensure that all of the components that provide case management and care coordination are reflected in the results.   A middle or senior manager would be the best person to complete the survey for your organization because he/she has a “birds’ eye” view of the organization.  He/she is familiar with client demographics, and knows the systems by which care is managed in your organization.  For Survey go to blocked::https://www.surveymonkey.com/s.aspx?sm=BLffq4jfc3p7QkMcRRtiWg_3d_3d">https://www.surveymonkey.com/s.aspx?sm=BLffq4jfc3p7QkMcRRtiWg_3d_3d  David KellyExecutive DirectorOntario Federation of Community Mental Health and Addiction Programs250 Consumers Road, Suite 806,Toronto, Ontario.M2J 4V6
